Online
Download and fill out forms on your computer.

At the Library
A limited number of Federal Tax Form 1040 Instruction Booklets and Forms and Ohio Individual and School District Income Tax Instruction Forms will be made available to the public as they arrive during the coming months in the lobby.
You may also print copies of the instructions or forms you need at any of our library computers (up to $10 a week for free using your library card). Ohio Individual and School District Tax Instruction Booklets will be available for $1.00 each at the Reference Desk.
Note: Some forms can only be obtained directly from the IRS, including the 1099, W-2 and W-4 forms. Place an order.
